The project “Implementation of the Presidio Nuovo Regina Margherita in Rome: Pilot Testing of the Casa della Salute Model” aimed to implement and validate an integrated, digitally enabled community care model within ASL Roma 1, strengthening continuity between hospital, community, home-based care, and prison healthcare services. The major outcomes included the integration of prison healthcare with territorial services through telemedicine, the evaluation and improvement of electronic health record usability, and the adoption of scalable teleconsultation and teleassistance platforms. The project also defined robust process and outcome indicators to support monitoring and quality assessment. Overall, it delivered a replicable, patient-centered model of integrated and data-driven care aligned with national and European digital health strategies.
